FOR CERTIFICATION (first time applicants) OR AMIAS RECERTIFICATION (already a sponsor and want to continue serving as an ALATEEN SPONSOR.

Bring food to share. Drinks will be provided.
If you have Alateen literature, please bring it with you.
A basket will be passed but there will be no fees to attend.
Space is unlimited but would be helpful if you will please RSVP no later than May 9th so we can have the workshop packets ready.
SPECIAL INSTRUCTIONS:
BRING A CHECK OR MONEY ORDER FOR YOUR BACKGROUND CHECK IN THE AMOUNT OF $6.64 ISSUED TO ETAAA. The background check form needs to be notarized, but do not worry I can notarize it for you at the end of the training. Do not forget to bring your photo ID for this form.
